Location: 1111 N. Nagle St P.O. Box 190 Alturas, CA 96101

Pay Rate: $31.87/hr on W2

Shifts: Days, Nights, Weekends

Job Summary:

We are seeking a compassionate and dedicated Certified Nursing Assistant (CNA) with an active California CNA license to join our Skilled Nursing Facility (SNF) team. The CNA will work closely with nursing staff, residents, families, and interdisciplinary teams to deliver high-quality, individualized patient care. The ideal candidate is a strong team player with excellent communication skills and a commitment to resident well-being.

Key Responsibilities:

• Receive and provide detailed shift reports on residents’ conditions, ensuring continuity of care
• Perform routine rounds and monitor residents’ overall health and safety
• Assess new admissions and observe residents who are ill, injured, or have experienced falls
• Accurately document resident assessments and care in accordance with facility policies
• Promptly report any changes in resident condition or care needs to nursing staff
• Assist with activities of daily living (ADLs), including hygiene, mobility, and feeding
• Ensure timely response to call lights and resident requests
• Maintain a safe, clean, and supportive environment for residents
• Collaborate effectively with nursing staff, physicians, families, and other departments
• Promote positive relationships with residents, families, and team members
• Float to other units as required to support patient care needs
• Perform additional duties as assigned

Required Qualifications:

• Active California Certified Nursing Assistant (CNA) License (Required)
• Current CPR/BLS certification (preferred or required per facility)
• Previous experience in a Skilled Nursing Facility (SNF) or healthcare setting preferred
• Strong communication and interpersonal skills
• Ability to work collaboratively in a team environment
• Compassionate, patient-focused approach to care
• Flexibility to work weekends, holidays, and varying shifts

Preferred Skills:

• Experience with electronic medical records (EMR)
• Strong observational and documentation skills
• Ability to handle multiple tasks in a fast-paced environment
